PromptTier

One layer of the prompt override cascade (§PM-3).

Detailed description

Tiers are ordered highest-priority first. Exactly one tier in a stack is writable (the scope the manager edits); the rest are read-only fallbacks (a lower on-disk tier, or the factory qrc bundle). name is the provenance label shown in the UI badge ("Factory" / "App" / "Project").
