Detailed description

`fleet.run` — start a program on machines: a run set (fleet §FLE-3, §FLE-8).

On existing instances it gates as `operational`. Given a `count` to buy, it starts a recurring charge, so it gates as `fleet-spend` and asks at every call, as `fleet.scale` does: the per-call hooks of OpalTask.
